چکیده

A 32-year-old man underwent catheter ablation of an orthodromic atrioventricular reentrant tachycardia. The sinus rhythm electrocardiogram exhibited a normal PQ interval and no delta waves, but atrial pacing produced a prolonged PQ interval and wide QRS morphology with right bundle-branch block due to antegrade accessory pathway (AP) conduction. During the tachycardia, atrial double potentials consisting of the coronary sinus musculature (CSM) and left atrial (LA) potentials were observed. Ventricular extrastimulation exhibited retrograde decremental conduction with an identical atrial activation sequence as during the tachycardia. A radiofrequency application within the posterolateral CS during ventricular pacing eliminated the CSM-LA conduction and concomitantly the ventriculoatrial conduction via the AP was abolished. In this case, the CSM was associated with the bidirectional decremental conduction properties of the AP, and the antegrade slow conduction resulted in the absence of a shortening of the PQ interval and delta waves during sinus rhythm despite the continuous presence of antegrade AP conduction.

Anatomic and electrophysiologic substrate of the permanent form of junctional reciprocating tachycardia.

Data are reported on three patients with the permanent form of junctional reciprocating tachycardia, in whom conduction over a slow accessory pathway was observed after His bundle ablation. Tachycardia was almost incessant and showed a retrograde P wave (P') and RP' interval longer than P'R interval in all patients; during sinus rhythm, the PR interval was normal and there was no evidence of a ...

Antegrade slow bypass conduction after closed-chest ablation of the His bundle in permanent junctional reciprocating tachycardia.

A case of permanent junctional reciprocating tachycardia in a 36-year-old woman successfully treated with closed-chest interruption of the His bundle is reported. Tachycardia had lasted for 14 years and showed a retrograde P wave (P') and RP' longer than PR' interval. The tachycardia used an anomalous pathway with a long conduction time in the retrograde direction. The atrial end of the anomalo...

Linking in accessory pathways. Functional loss of antegrade preexcitation.

BACKGROUND Concealed retrograde activation has been proposed as a mechanism for antegrade conduction block in the bundle branches and atrioventricular accessory pathways. We studied this hypothesis (linking) in 10 patients with the Wolff-Parkinson-White syndrome in whom antegrade preexcitation could be persistently blocked by overdrive atrial pacing. METHODS AND RESULTS An atrial pacing proto...

Mahaim fibre coexisting with anomalous origin of the left main coronary artery from the right aortic sinus and an interarterial course.

We present the case of a 45-year-old female with a normal heart, who exhibited an episode of a broad QRS tachycardia that was initially suggested to be sustained ventricular tachycardia. Coronary angiography showed the left main stem originating from the right aortic sinus with an interarterial course between aorta and right ventricular outflow tract. Electrophysiological study revealed the pre...

Radiofrequency ablation therapy in concealed left free wall accessory pathway with decremental conduction.

An electrophysiologic study followed by transcatheter radiofrequency ablation therapy was performed in two adult patients with a permanent form of junctional tachycardia. Both patients had no structural heart disease and exhibited a normal resting ECG. The P wave during tachycardia was negative in leads 1, 3, and aVF, biphasic over V6, and positive in V1 and aVL in both patients, while the P-R/...
